Topiramate use in pregnancy: MHRA safety guidelines tightened

Published: June 24 2024
Last updated: July 25 2024

Kami Kountcheva | The MHRA has updated its safety guidelines for the use of the medicine topiramate to help prevent harm to unborn babies

Topiramate should not be prescribed to women of childbearing potential without a pregnancy prevention plan, the Medicines and Healthcare products Regulatory Agency (MHRA) has announced.

The MHRA updated its safety guidelines for the use of the medicine topiramate (brand names Topamax and Topiragen) on 20 June 2024. Topiramate is used to treat epilepsy and migraines.

Previous MHRA guidelines from May 2022 said that before being prescribed topiramate, women should be fully informed of the risks during pregnancy, and in epilepsy, alternative options should be considered first.

The new guidelines state that topiramate should not be given to “women of childbearing potential unless the conditions of a Pregnancy Prevention Programme are fulfilled”.

They continue to stress that women must be made aware of the risks of topiramate and now need to sign the Risk Awareness Form.

The update follows a review conducted by the MHRA, which found that “the use of topiramate during pregnancy is associated with significant harm to the unborn child”.

The MHRA said the medicine could lead to birth defects and low birth weight. It said that it could also increase the risk of intellectual disability, autistic spectrum disorder (ASD) and attention deficit hyperactivity disorder (ADHD) in children.

Advice from the MHRA says that women should always use effective birth control while taking topiramate and four weeks after stopping the medicine. Topiramate can change how effective some hormonal types of birth control are, so the MHRA advises women to speak to their GPs or specialists about effective contraception.

The MHRA said women with epilepsy taking topiramate, who are thinking about having a baby or think they might be pregnant should continue to take their epilepsy medicine and make an urgent appointment with their GP or epilepsy team.

Stopping an epilepsy medicine could lead to seizures restarting or becoming more frequent or more severe.

Reduced medication options for women

The latest NHS England statistics show that in September 2023, the number of females aged under 55 years old who were prescribed topiramate was just over 30,000.

Alison Fuller, director of health improvement and influencing at Epilepsy Action, said: “Information is always key.

“From some of our previous research, we know not enough women were being fully informed about the risks some anti-seizure medications pose to pregnancy. The inclusion of topiramate in the Pregnancy Prevention Programme should at least ensure that these conversations are happening.

“Clinicians need the right support to make sure that the change in rules is effectively communicated to affected patients, and that they can make informed decisions about their care.

“While these new rules coming into effect may be concerning for some people, it’s important no one stops taking medication without speaking to their specialist.

“We understand that these new measures could potentially reduce medication options for women of child-bearing age. Also, we know that neurology staffing levels are extremely low in the UK, and new referrals could increase the already critical waiting times.

“We will monitor the roll-out closely and keep sharing any new information with our supporters as any developments occur.”

“No information and no idea”

Epilepsy Action, Young Epilepsy and Epilepsy Society surveyed more than 1,200 women and girls with epilepsy in 2022 about epilepsy medicines in pregnancy. The survey showed that more than a quarter of respondents taking topiramate (28%) didn’t know about the risks of the medicine in pregnancy.

One responder taking topiramate said: “I had no idea any of these medications could be harmful.”

Another commented: “When I was first diagnosed, I was 20. I had no information and no idea about epilepsy and pregnancy. I think part of the reason I haven’t had children is because I was afraid of the effects of my tablets on a baby.”

Some responders had already been through pregnancy. One said: “It was mentioned when I was first diagnosed that some medications wouldn’t be suitable if I wanted a child, but my neurologist didn’t tell me which ones would put my baby at risk. I was on topiramate and carbamazepine when my child was born seven years ago (I didn’t know I was pregnant until I was in labour) and I had to be checked over. Luckily my child is okay, however, he’s on the waiting list to be screened for autism and ADHD.”

Another responder shared her experience, saying: “I’ve successfully had two safe pregnancies and healthy children with advice from my neurologist and epilepsy nurse.

Sabril (Vigabatrin) recall

Published: July 14 2023
Last updated: July 21 2023

14 July 2023

The Medicines and Healthcare products Regulatory Agency (MHRA) has issued a medicines recall affecting some Sabril tablets and granules. Sabril is the brand name for the epilepsy medicine, vigabatrin.

This measure is being taken is as a precaution due to traces of another medicine called tiapride being detected in some batches of the ingredients used to make Sabril. The MHRA highlight that Tiapride is currently not licensed for use in the UK, and it’s use in children has not been thoroughly investigated.

It’s important not to stop taking Sabril tablets or granules without speaking to a doctor. The MHRA say that the risks of suddenly stopping epilepsy medicine are higher than the potential risk from tiapride.

Advice for adults

The MHRA say that people over 18 with batches of the affected medicine do not need to return them for a replacement and can continue to take their medicine as normal. The trace amounts of tiapride are significantly lower than the safe limits for adults.

Advice for children and parents

People aged 17 or below with batches of the affected medicine, or their parents, should return them to their pharmacy for replacement. If medicine from an affected batch has been used, the MHRA advises that you should contact your doctor but keep taking your medication until you have an alternative batch, where possible.

Batches affected:

Sabril 500mg film coated tablets (product code: PL 04425/0171)

  • Batch number 2989A with expiry date of April 2025

  • Batch number 2988D with expiry date of April 2025

  • Batch number 2006B with expiry date of November 2025

Sabril 500mg granules for oral solution (product code: PL 04425/0170)

  • Batch number 1994A with expiry date of May 2024

  • Batch number 2028B with expiry date of March 2025

Where to get further advice

If you’re unsure if your medicine is affected, experience side-effects or have any other concerns, please speak to your GP or pharmacist for further advice.

Charities and pharmacists are calling for a complete review of the medicine supply chain as shortages continue to put people’s health at risk.

Charities and Association of Independent Pharmacies have warned the government that unless there is greater openness with drug manufacturers, medicine shortages will continue to cause harm to patients.

A survey from the charities found 70% of respondents had difficulties getting hold of medication in the past year. The same survey found 22% had experienced problems in the last month.

More than half (55%) needed to visit multiple pharmacies before getting hold of their medication, and two-thirds were only given a fraction of their prescription.

The Association of Independent Multiple Pharmacies added that community pharmacists were spending two hours every day trying to track down potentially life-saving medications for patients.

The charities and pharmacists are calling for an urgent meeting with the UK health secretary, Victoria Atkins.

The journey to finding the right medication is often a tough one for people with epilepsy. When they do find it, they should not have to fear the security it gives being taken away by factors totally out of their control.

“They shouldn’t have to be anxious about running out of their medication and not being able to find the next dose. They shouldn’t have to travel miles and miles to find it. They shouldn’t have to worry about switching to a different one that could bring side effects, not keep their seizures under control, or even cause a breakthrough seizure when they’ve not had one for years.

We need clarity on the causes and a straight answer on how this issue is going to get tackled, and this is why charities and associations in calling on the government to take action, now.”

The medicines supply chain is broken at every level and unless the Department of Health reviews its processes and procedures, we will never achieve the stability that will guarantee patients their prescription when they need it.

“The UK has a smaller medicines budget than anywhere else in Europe or the US. The NHS is continually driving down the price of medications, which means that manufacturers prefer to sell outside of the UK where they can make a better profit.

“The system is overly complex and shrouded in secrecy – what we need is openness and transparency. Pharmacists are in the same position as patients – we are at the end of the supply chain but are the last people to find out about medication shortages. Consequently, we are unable to plan in advance and support the people who rely on us for their medications.”

Responding to the calls, a Department of Health and Social Care spokesperson said: “We have taken swift action with NHS England and other stakeholders to improve the supply of epilepsy medications and access to some has already improved while we expect supplies of others to improve in the coming months.

Valproate boxes must now contain safety warnings

Published: October 18 2023

Last updated: October 18 2023

The MHRA has updated its guidance on dispensing valproate-containing medicines.

The medication will now be dispensed in the manufacturer’s original full packaging.

The change was made to ensure that patients receive safety warnings, including a patient card and information leaflet.

Sodium valproate is commonly prescribed to treat epilepsy and also sold under the names: Epilim, Epilim Chrono, Epilim Chronosphere MR, Episenta, Episenta MR, Epival CR, Dyzantil MR and Depakin.

The move follows concerns that some people taking the medicine were unaware of the risks to an unborn baby if valproate-containing medicines are used in pregnancy.

While many people were already receiving the medication in its original packaging, some were receiving it without the warnings because the prescribed amount of medicine was different to the amount in the full pack.

In rare cases, pharmacists will be able to make an exception to the requirement, but only where a risk assessment is in place that refers to the need for different packaging.

The change came into force in England, Scotland and Wales on 11 October.

In 2018, a review by the MHRA led to the introduction of the Valproate Pregnancy Prevention Programme, which is now a condition of prescribing and dispensing valproate-containing medicines to women of childbearing potential.

MHRA chief executive Dr June Raine said: “It is essential that all patients on valproate-containing medicines receive the latest safety information every time their prescription is dispensed. The changes in the law announced today ensure this happens. People’s situation may change, especially with regard to the possibility of pregnancy, so it is vital that the warnings about the harms of valproate are always brought to mind

5 Nov2019

Update 5 November 2019: Pfizer has been in touch to say Epanutin Infatabs are still out of stock and they are expecting a further manufacturing delay. Pfizer has ordered extra supplies of Dilantin Infatabs from Canada to cover the shortfall until mid-2020.

Update 3 May 2019: Pfizer now expects Epanutin Infatabs will be back in stock in November 2019.

While Epanutin Infatabs are out of stock, Pfizer will be importing a Canadian version of chewable phenytoin tablets called Dilantin Infatabs. The active ingredient is the same, but there may be differences between Epanutin and Dilantin that for some people could affect seizure control and/or side-effects. If you need to change from one version of phenytoin to another this should be closely monitored by your doctor. This monitoring may involve doing blood tests to check that you have the right levels of phenytoin in your blood.

If you take Epanutin Infatabs we would advise you to talk to your GP or epilepsy specialist  as soon as possible. This is in case any changes need to be made to your treatment while the medicine is out of stock

20 Aug 2019

Update: 19 August 2019
We have heard from Teva, the makers of Gabitril, that Gabitril will not be back in stock before the end of October. This further delay is caused by a problem with part of the manufacturing process. This is a Europe-wide issue.

Teva have also advised us that the same manufacturing problem may mean that Gabitril 10mg could experience a disruption in supply in early 2020.

If you are due to run out of Gabitril before the end of October we would advise contacting your epilepsy specialist as soon as possibleurgently. This is so you can discuss as soon as possible, what to do as soon as possible before you run out.

Update: 1 August 2019

The Department of Health and Social Care (DHSC) has confirmed that Gabitril is out of stock due to a manufacturing issue that is affecting the whole of Europe.

The DHSC has reached out to medicine importers to see if supplies of tiagabine can be sourced from overseas. However, as the manufacturing issue affects the rest of Europe, supplies may not be readily available.

If you are due to run out of Gabitril before the end of September we would advise contacting your epilepsy specialist as soon as possible. This is so you can discuss what to do in plenty of time before you run out.

Epanutin oral suspension shortage in the UK

7  June 2019

Pfizer, the makers of Epanutin (phenytoin), have told us there will be a temporary shortage of Epanutin 30mg/5ml oral suspension from the week beginning 10 June 2019. This is due to a manufacturing delay. Pfizer expect it will be out of stock until the end of July 2019.

The Department of Health & Social Care (DHSC) has sent an alert to healthcare professionals about this supply issue. They have asked GPs to make early contact with all their patients who are prescribed this medicine, to check if they have enough to last them until the end of July. If you don’t have enough supplies, you may wish to contact your GP now, rather than wait to hear from them.

While Epanutin oral suspension is out of stock, Pfizer will be importing a Canadian version of phenytoin oral suspension called Dilantin-30. The DHSC has advised GPs to switch patients who don’t have enough Epanutin oral suspension to Dilantin-30, until Epanutin is back in stock. Dilantin-30 contains the same active ingredient, and the DHSC says it can be considered equivalent to Epanutin. If you or your GP do not feel Dilantin-30 is appropriate for you, your GP should refer you to a specialist for advice.

You should not need any changes to your dose or frequency of your medicine when taking Dilantin-30. However, the Canadian patient information leaflet that comes with Dilantin-30 has different dosing information compared to the Epanutin leaflet. It’s important to follow your doctor’s instructions for taking Dilantin-30, even if these are different from the dosing information on the leaflet.

Changes to Keppra (levetiracetam) dispensing in the UK

21 May 2019

UCB, the makers of Keppra, have announced changes to the way pharmacies dispense Keppra. The changes mean that some people who usually get Keppra from their pharmacist will be given generic levetiracetam instead.

Like many medicines, levetiracetam is available in a branded version (Keppra) and also in a number of generic versions made by different manufacturers. With most medicines, pharmacists are only able to give you the branded version of a medicine if your doctor writes the brand name on the prescription. This is because the branded version usually costs more to the NHS than the generic versions. However, since 2011 UCB has had schemes allowing pharmacies to dispense Keppra even if the brand name is not written on the prescription. This was at no extra cost to the NHS.

Due to recent changes to some of the schemes UCB operates with pharmacies, some pharmacies that have previously dispensed Keppra when given a generic prescription will no longer do so.

I have Keppra written on my prescription. Will this change affect me?

This change should not affect you. If the brand name Keppra is written on your prescription, your pharmacist must continue to give you Keppra.

My prescription just says levetiracetam, but my pharmacist usually gives me Keppra. Will this change affect me?

This will depend on whether or not your pharmacy is affected by the change. Some pharmacies will continue to dispense Keppra with a generic prescription, but UCB has not revealed which ones for commercial reasons. You may wish to speak to your pharmacist to see if they are affected by the change.

What can I do if I want to stay on Keppra, but my pharmacist says they need to start giving me generic levetiracetam?

Some pharmacies will still be able to dispense Keppra, so one option is to take your prescription to a different pharmacy. As UCB has not revealed which pharmacies are affected by the change, we’re not able to say which pharmacies to try

Sanofi statement about Epilim and Depakote stock levels
7 Feb 2019
In response to concerns about stock levels of Sanofi valproate-based medicines (Epilim and Depakote), Sanofi has issued the following statement:

Sanofi valproate out of stock statement – February 2019
Sanofi can confirm that supply of valproate (Epilim and Depakote) is available in the UK. Any patients who have concerns should contact their pharmacist who can contact the Sanofi customer service team so that we can assist and help resolve the issue.

Due to temporary disruption in the production of valproate at a Sanofi manufacturing site outside of the UK, stock levels for some treatments of valproate may be lower than usual. Sanofi is managing the situation carefully and working to ensure continued supply of valproate across all markets, including the UK. There are 17 different valproate-based treatments, ranging from tablets of different strengths and coatings through to granules and liquid preparations. We have notified the Department of Health & Social Care about the disruption of supply of valproate.

This temporary disruption in supply of some valproate-based treatments is not related to Brexit.

Change from 100 tablet packs to 30 tablet packs: At the request of the Medicines and Healthcare products Regulatory Agency (MHRA) pack sizes of valproate have been changed from 100 to 30 tablets. Pharmacies may therefore see that the 100 tablet packs are “unavailable” and assume that valproate is out of stock. The codes for 30 tablet packs should be used. The Customer Services line is available for pharmacists to check on stock levels and ordering codes.

This is due to manufacturing issues. Mylan does not expect to return to normal stock levels until the middle of 2019. We will update this page as soon as we know more.
